Responsibilities:

Provides skilled nursing services as per care plan.
Documents patient observations, interventions, and goals.
Delivers and documents patient care during visits.
Communicates changes in patient condition to the clinical team.
Submits documentation as per agency requirements.
Meets productivity standards and clinical competencies.
Uses agency-provided telecommunication devices appropriately.
Follows standards for technology and documentation.
Engages in education, quality improvement, and protocol development.

Qualifications:

Licensed Vocational Nurse License in California
Current CPR/ BLS Certification
Physical and TB tests (within one year)
Driver's License or State ID
IV Certification
Home Health experience is a plus
Ability to drive within 30-60 miles a day
Reliable transportation and insurance
