Job Summary:
The Registered Dietitian is responsible for providing nutrition care and education to PACE participants. This role involves evaluating participants' nutritional status, developing and implementing individualized nutrition care plans, and working collaboratively with the interdisciplinary team to ensure optimal health outcomes. The Dietitian will assess nutrient requirements, recommend appropriate dietary prescriptions, and monitor the effectiveness of nutritional interventions while ensuring compliance with PACE regulations and standards of care.
Key Responsibilities:
1.Nutritional Assessment and Care Planning:
•Conduct initial comprehensive nutritional assessments for each participant, evaluating their health status and dietary needs.
•Develop and implement individualized nutrition care plans as part of the interdisciplinary team (IDT) approach.
•Perform periodic reassessments at least semi-annually, or more frequently based on participant condition or requests.
•Provide recommendations for nutritional prescriptions and assess participants' tolerance to interventions.
2.Collaboration with Interdisciplinary Team (IDT):
•Regularly inform the IDT of participants' medical, functional, and psychosocial conditions.
•Actively participate in the development and updating of care plans.
•Communicate nutritional care information effectively to physicians, nursing staff, and other healthcare professionals.
3.Nutrition Education and Counseling:
•Provide nutrition education and counseling to participants and their caregivers in alignment with the plan of care.
•Educate PACE staff and the community on nutrition-related topics, ensuring up-to-date dietary practices.
•Develop and deliver educational materials or presentations based on participant needs and clinical guidelines.
4.Documentation and Compliance:
•Maintain thorough documentation of nutritional assessments, care plans, and interventions in participants' medical records in compliance with PACE policies and procedures.
•Ensure that changes in participant condition are recorded promptly and communicated with the team.
•Follow all relevant regulatory requirements as outlined in 42 CFR Part 460.104 (Assessments) and adhere to ADA standards of practice.
5.Health Promotion and Disease Prevention:
•Monitor and address participants' chronic conditions, such as diabetes or cardiovascular disease, through dietary interventions.
•Work with participants to prevent malnutrition, promote weight management, and improve overall health outcomes through targeted nutrition strategies.
Qualifications:
• Education:
•Bachelor's degree in Nutrition or Dietetics from an accredited institution.
•Must be a Registered Dietitian (RD) with current registration from the Commission on Dietetic Registration (CDR).
•Certified Diabetes Educator (CDE) status preferred, or CDE eligibility within 24 months.
• Experience:
•Minimum of one year of experience working with frail and/or elderly populations, preferably in a healthcare or PACE setting.
• Skills:
•Strong knowledge of clinical dietetics and medical nutrition therapy.
•Ability to work within an interdisciplinary team and communicate effectively with healthcare providers.
•Analytical thinking and the ability to assess and interpret nutritional data.
•Proficiency in electronic medical record (EMR) systems and dietary software.
Physical Requirements:
• Ability to sit for extended periods, use office equipment, and communicate effectively in person and over the phone.
• Occasional travel to PACE centers or training events may be required.
